This happens when the EjbcaMail service is NOT configured in the application server.
On Managing / Issuing CA
1. Create an approval profile with notifications enabled
2. Select it for "CA Activation" approval in a CA.
3. deactivate the CA
4. Reactivate with the valid passcode
5. Login as a different user
6. Goto Supervision Functions / Approve Actions
7. Select CA for approval
8. Select Approve and click Save State
ACTUAL: ServiceLocatorException, see screenshot.
EXPECTED: CA to be reactivated.
